In TRT the term "directive counseling" means education. You must learn how tinnitus works in our brains. Learn how our auditory system, emotional system, and the system that puts our body in a state of alert, are strongly inter-connected. Learn that this inter-connection is responsible for boosting the tinnitus sound and consequently most of the suffering that the tinnitus sound leads to.

Directive counseling is the most important part of TRT. Sound therapy seldom has any effect on habituation if directive counseling is skipped. Also, having negative feelings about using sound therapy makes the sound therapy worthless.

The main reason why directive counseling is so important? The brain cannot get used to (habituate to) a sound that is unknown (might indicate danger) and a sound that is categorized as dangerous or strongly negative. In order to facilitate habituation, the sound has to be recognized as unimportant (i.e. not a sign of danger).

If you cannot come to the conclution that the tinnitus sound is an unimportant sound, it helps a bit to reduce the aversive feelings you have against the tinnitus sound. By knowing a lot about how the brain handles the tinnitus sound, aversion will decrease in most people. And the less negative attitude to tinnitus, the easier habituation.
